We will be accepting applicants for school year 2009-10, and we will give special preference to brothers and sisters of our students, as well as, to children of Dasmarińas Village residents.

 

NOTE :

·         LEVELS OPEN FOR APPLICATION:                         NURSERY TO HS

·         FOR PRESCHOOL NEW STUDENTS:                      AM SESSION on a first-come, first-served basis

·         APPLICANTS WITH NO/MISSING DOCUMENTS will not be entertained.

FOR PRESCHOOL

PROCEDURE:

 WHEN APPLYING, BRING:

 

×          See the Preschool Secretary for the Appointment Slip and show the Birth Certificate for age confirmation.

×          Proceed to the Accounting Office for payment of Application Fee.

×          Present the Receipt to the Preschool Secretary for interview schedule.

×          Bring the applicant to the Guidance Center on testing date.

×          Visit the Preschool Department for test result one week after testing date.

×          Fill out and then submit Application Form to the Preschool Department.

×          Get the Enrollment Slip from the Registrar’s Office and fill it out completely and accurately.

×          Pay the Initial Fees at the Accounting Office.

×          Bring the Receipt and the Enrollment Slip to the Registrar’s Office and get the Registration Form (proof of complete enrollment)

×          Go to the Preschool Department to secure the list of reminders on books, materials, uniforms, etc.

                         

·        Birth Certificate  (clear photocopy)

·        Preferably from the National Statistics Office, East Ave., Diliman, Q.C., or from City Hall with registry number  (if Filipino)

·        From the Office of the Civil Registry of the country of birth with official English translation or Embassy Certification of birth details (if born abroad)

·        Report Card (photocopy of the latest) or Certificate of Attendance (if no report card)

·        Application/Inteview/Testing Fee (P700)

WHEN ENROLLING, SUBMIT:

e        Birth Certificate  (clear photocopy)

e     Preferably from the National Statistics Office, East Ave., Diliman, Q.C., or from City Hall with registry number (if Filipino)

e   From the Office of the Civil Registry of the country of birth with official English translation or Embassy Certification of birth details (if born abroad)       

e        Report Card (photocopy of the latest) or Certificate of Attendance (if no report card)

e        4 ID Pictures (1 x 1)

e        Passport/Special Study Permit  (if foreigner)

FOR GRADE SCHOOL

 

PROCEDURE:

WHEN APPLYING, BRING:

Ř     If from a local school, see the GS Secretary to submit requirements and to get the Application Form.

Ř     If from a foreign school or if a returning student:

·       see the Registrar’s Office personnel, first, for grade   evaluation,

·       then proceed to the GS Secretary to get the Application Form.

Ř     Fill out the Application Form completely.

Ř     Proceed to the Accounting Office for payment.

Ř     Show the Receipt to the Guidance Center for test schedule and Recommendation Form. (For Gr. 1 applicant, the pupil must be present for interview.)

Ř     Come on testing day and submit the Recommendation Form to the person administering the test.

Ř     Come back and see the GS Secretary on the day the results are to be released.

×          Fill out the Application Form completely.

×          Show the Receipt to the Guidance Center for test schedule.

 

×          Come on testing day and on the day the results are to be published.

 

·        Birth Certificate  (clear photocopy)
